We make every effort to decrease the cost of your medical care, and therefore, request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. We accept cash, checks, and most credit cards for your convenience. We will bill your insurance company; however, payment is the patient's responsibility. We will help in every way we can to assist in handling claims.



We are members of most area Preferred Provider organizations and accept Medicare Assignment. Worker's Compensation and No-Fault insurances are accepted. If your insurance has a co-payment, we would appreciate payment at time of visit. We accept cash, checks, and major credit cards for your convenience.


Please request all prescriptions and authorizations for renewals during normal office hours when our full records are available.
